This is obviously a work in progress, but here is the basic layout of both the Soul Society and Seireitei.
Each number above pertains to the Division of the respective numbered Squad. For instance, 5 denotes the 5th Divisional grounds. Letters denote specific areas (Players whom hold active Shinigami may propose areas to be added):
